This evaluation report has two purposes. The first purpose is to provide information about the student’s performance, and the second is to inform the student of her/his progress to date at the placement organization.  On the following pages, we provide a guide for evaluating the student’s progress. We suggest that you respond to the questions by giving examples to illustrate your assessment. The report may also be discussed at meeting with the faculty advisor.
