Transportation Assistance

Are you attending a New Directions program through Canadian Mental Health Association and in receipt of O.D.S.P.? If so, transportation funds may be available to you.

Simply take the "Mandatory Special Necessities Form" (which can be mailed to you by your O.D.S.P. worker) to your doctor and have him/her complete the form and return it to O.D.S.P.
